Terms and Conditions for NDIS Disability Service Provider
1. Introduction

2. Eligibility
Our services are available to individuals eligible for the National Disability Insurance Scheme (NDIS). You must provide valid documentation confirming NDIS eligibility, including NDIS plans, funding details, and any relevant medical or support documentation.
3. Service Agreement
Upon enrolling in our services, you will enter into a formal Service Agreement outlining the specific services we will provide, including frequency, duration, and costs. The Service Agreement will be tailored to your individual needs and goals and will comply with NDIS regulations. You are responsible for reviewing and understanding the Service Agreement before accepting.
4. Service Delivery
We are committed to delivering high-quality, person-centered services. However, service availability may vary depending on staff availability and location. We will make every effort to deliver services as per the agreed schedule. Any changes to service delivery will be communicated to you in advance, where possible.
5. Cancellation and Rescheduling
You must provide at least 24 hours’ notice if you need to cancel or reschedule a service. Failure to provide sufficient notice may result in a cancellation fee, as outlined in your Service Agreement. We reserve the right to cancel or reschedule services in cases of unforeseen circumstances, including staff illness, emergencies, or other factors beyond our control.
6. Fees and Payments
Service fees are determined based on NDIS funding and the services outlined in your Service Agreement. All payments must be made in accordance with NDIS guidelines. If you are self-managing your NDIS funds, invoices will be sent to you directly, and you are responsible for timely payment. Late or missed payments may result in a suspension of services.
7. Participant Responsibilities
You agree to:
- Provide accurate and up-to-date information regarding your NDIS plan and needs.
- Engage with your support team to ensure services are delivered effectively.
- Inform us of any changes to your circumstances, such as a change in NDIS funding or personal contact information.
- Treat our staff with respect and courtesy at all times.
8. Confidentiality and Privacy
We are committed to protecting your privacy and ensuring the confidentiality of all personal and sensitive information you provide. All data collected will be used solely for the purpose of providing NDIS services and will be handled in accordance with our Privacy Policy and applicable privacy laws.
9. Complaints and Feedback
Your satisfaction is important to us. If you have any complaints or feedback regarding our services, you can submit them through our official complaints procedure. We are committed to addressing concerns promptly and fairly and will strive to resolve any issues in accordance with NDIS guidelines.
10. Liability
We are not liable for any direct, indirect, or consequential damages arising from the use of our services, except where such liability is mandated by law. 11. Termination of Services
You or Care Blooming may terminate the Service Agreement at any time, with reasonable notice, for any reason. Termination may be initiated due to changes in NDIS funding, repeated non-compliance with terms, or failure to maintain a safe working environment. Upon termination, you will be responsible for settling any outstanding payments.
12. Amendments to Terms
We reserve the right to amend these Terms and Conditions at any time. Any significant changes will be communicated to you in writing, and your continued use of our services will signify acceptance of the revised Terms.
13. Governing Law
